Job summary

Wi Don Council in Appleton, Wisconsin is seeking an experienced MDS Coordinator to oversee the federally mandated resident assessment process while coordinating care plans compliant with state and federal guidelines. This role involves working with an interdisciplinary team, ensuring compliance with the RAI process, and supporting Medicare documentation.

Ideal candidates will have a Bachelor’s in Nursing and valid RN licensure, with direct MDS experience strongly preferred. This position offers a competitive salary and great benefits including health insurance and retirement plans.

Qualifications

• Direct MDS Coordinator experience preferred.
• Current certification in adult CPR required.
• RAC-CT certification preferred.

Responsibilities

• Coordinate the RAI process and ensure regulatory compliance.
• Develop and revise comprehensive care plans.
• Maintain compliance with state-specific regulations.
• Coordinate care with case managers and health agencies.

Skills
