Job summary

Apploi NW Montana is seeking an MDS Coordinator to join our nursing team. You will coordinate Medicare/MDS assessments, work with the IDT, ensure timely documentation, and support the plan of care under the Director of Nursing Services.

The role requires active nursing licensure (RN or LPN), strong English communication, and solid administrative and organizational skills to maintain compliance with federal and state regulations and company policy.

Qualifications

• Current licensure in nursing.
• RN or LPN licensure required.
• Written and verbal communications in English as business necessity.
• Administrative and organizational abilities are essential.

Responsibilities

• Notify and coordinate Interdisciplinary Team (IDT) for MDS assessments.
• Coordinate Medicare/MDS resident assessment process.
• Ensure timely completion of MDS assessments by IDT.
• Coordinate development, implementation, and evaluation of the care plan.
• Provide nursing care and interventions as indicated by the plan of care or physician orders.
• Maintain infection control standards in nursing care.
• Adhere to safety policies and company policy.
• Manage on-call duties to meet resident needs.
• Ensure accurate documentation and information exchange for quality care.
• Participate in meetings and inservices as assigned.
• Protect resident rights and resolve consumer concerns.
