Job summary

The MDS Float Position at Naya Healthcare seeks an experienced MDS Coordinator to support three facilities (two in Frostburg, one in Cumberland). You will review records, coordinate with care teams, and ensure timely MDS assessments.

Responsibilities include maintaining detailed care plans, communicating with physicians and families, and complying with regulatory guidelines while managing multiple tasks in a fast-paced environment.

Qualifications

• At least 1 year of MDS experience.
• Point Click Care (PCC) experience is a plus.
• Ability to work independently and manage multiple tasks and schedules.
• Strong communication and interpersonal skills.
• Attention to detail and organizational skills.
• Ability to work long hours at a desk and fly solo on most days.

Responsibilities

• Conduct MDS assessments for patients in collaboration with the care team.
• Review medical records and document patient information accurately and thoroughly.
• Communicate with physicians, families, and other healthcare professionals to ensure comprehensive care.
• Maintain a detailed and organized plan of care for each patient.
• Ensure compliance with regulatory guidelines and company policies.
