Job summary

Westview Rehabilitation in Houston, TX is seeking an experienced MDS Coordinator (LVN/RN) to oversee patient assessments and regulatory compliance. The role requires MDS/PPS and PSSR experience, MDS 3.0 fluency, and strong knowledge of long-term care Medicaid, PCC, and related reporting systems.

The MDS Coordinator will lead interdisciplinary meetings, update care plans, manage MDS data transmission, and provide quality reporting to leadership.

Qualifications

• Minimum 2 years of long-term care experience.
• Must have MDS/PPS experience and PSSR experience.
• Fluent in MDS 3.0 and knowledgeable about Medicaid, MDS and PCC.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure compliance with Federal and State guidelines for MDS.
• Review MDS and care plans for accuracy and thoroughness.
• Maintain expertise in MDS software for data entry and reporting.
• Lead interdisciplinary meetings for Medicaid, ITTM, and care planning.
• Update and revise care plans based on current assessments.
• Generate and distribute Quality Indicators reports to leadership.
• Ensure timely transmission of MDS data per standards.
• Attend professional development to stay current with industry best practices.
• Report ancillary data to corporate billing for accurate processing.

MDS COORDINATOR

In this role, you will:

• Coordinate Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments.
• Ensure Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS).
• Monitor Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization.
• Utilize MDS Software: Maintain expertise in software systems to ensure correct data entry and reporting.
• Lead Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.
• Update Care Plans: Adjust and revise care plans as needed based on current assessments and resident status.
• Provide Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement.
• Submit MDS Data: Ensure timely and accurate transmission of MDS assessments per reporting standards.
• Stay Current: Attend professional development sessions to keep up with industry standards and best practices.
• Oversee Ancillary Data: Collect and report ancillary services to corporate billing for accurate processing.

Knowledge / Experience:

• Must have 2 years of long term care experience
• Must have MDS / TILE experience
• Must be fluent in MDS 3.0

Licensure / Certifications:

• Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in Texas.
