Job summary

RegalCare at Courtyard at Medford is seeking a qualified RN MDS Coordinator to join our team. The role involves coordinating the development and completion of resident assessments in line with state requirements and facility goals.

The successful candidate will manage MDS-related processes, update policies, and collaborate with care teams and managed care providers while supporting ongoing quality assurance and discharge planning activities.

Qualifications

• Licensed RN in the state of Massachusetts is required.
• Experience coordinating and completing MDS/PPS/PDPM assessments.
• Familiarity with CMS guidelines and resident assessment policies.

Responsibilities

• Coordinate and develop resident assessments (MDS/PPS/PDPM) per state rules and facility policies.
• Maintain and update policies governing resident assessments and care plans.
• Coordinate clinical updates to managed care providers.
• Perform administrative duties (forms, reports, evaluations).
• Develop and maintain an ongoing QA program for assessments and care plans.
• Assist with discharge planning related to care plans.
• Participate in surveys and monitor QA/QM reports.

Position Overview

We are currently seeking a compassionate and skilled RN MDS Coordinator. The RN MDS Coordinator is responsible for conducting and coordinating the development and completion of the resident assessments by the requirements of this state and the policies and goals of this facility.

Responsibilites

• Conduct and coordinate the development and completion of resident assessments (MDS/PPS/PDPM) by current rules, regulations, and guidelines that govern the resident assessment, including theCAAs.
• Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessments(MDS/PPS/PDPM) and care plan.
• Coordinate clinical resident updates to managed care providers.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, etc., as necessary.
• Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
• Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident’s discharge plan.
• Participate in facility surveys (inspections) made by authorized government agencies.
• Monitor the facility’s QI and QM reports ensuring that appropriate corrective action can be implemented when potential problem areas occur.
• Ensure that all assessments are completed and transmitted promptly. Report problem areas to the administrator.
• Ensure that a current copy of the MDS Instructor’s Manual is available to persons completing portions of the resident assessments.
• Review the facility’s Casper Reports with the facility administrator and Regional Director of Clinical Reimbursement to ensure proper follow-up as directed.
