Job summary

Complete Care is seeking an experienced MDS Coordinator/LPN to join our team on a full-time, day shift in Pennsylvania. The role focuses on overseeing resident assessments, guiding MDS input, and ensuring timely submission to state regulators.

You will collaborate with nursing leadership to maintain high-quality care and regulatory compliance. The ideal candidate has MDS and long-term care experience, holds an active PA LPN license, and demonstrates strong computer skills.

Qualifications

• MDS experience and LTC experience preferred.
• Active PA LPN license is required.
• Ability to work independently or as part of a group.
• Basic computer skills and familiarity with patient records.

Responsibilities

• Monitor and guide the completion of resident assessments in a timely manner.
• Involvement and supervision of inputting MDS assessments.
• Transmit MDS assessments to the state and obtain receipt of validation.
• Schedule and facilitate care plan conferences on a quarterly and annual basis.
• Other duties as assigned by Director of Nursing and Administrator.
