Qualifications

• Active Pennsylvania LPN license in good standing.
• Minimum of two years of MDS experience in a skilled nursing facility.
• Knowledge of MDS processes, care planning, and regulatory requirements.

Responsibilities

• Coordinate and complete MDS assessments per state and federal regulations.
• Ensure assessments are accurate and timely.
• Collaborate with nursing staff and interdisciplinary team for care planning.
• Update resident care plans based on assessment findings.
• Monitor assessment schedules and maintain documentation.
