Job summary

Complete Care, a locally trusted provider in PA, is hiring a MDS Coordinator/LPN for a full-time day shift. You will oversee resident assessments, guide MDS input, and coordinate state submissions with the nursing leadership.

Qualified candidates will have MDS and LTC experience, ability to work independently or with a team, and an active PA LPN license. Computer skills are required to manage documentation and care plans.

Qualifications

• MDS experience and LTC experience preferred.
• Ability to work independently or as part of a group.
• Computer knowledge required.

Responsibilities

• Monitor and guide the completion of resident assessments in a timely manner.
• Involvement and supervision of inputting MDS assessments.
• Transmit MDS assessments to the state and obtain receipt of validation.
• Schedule and facilitate care plan conferences on a quarterly and annual basis.
• Other duties as assigned by Director of Nursing and Administrator.
