Job summary

The Tremont Rehabilitation and Skilled Care Center in Wareham is hiring an MDS Coordinator to lead the MDS process and resident care plan assessments. The role supports CMS compliance, charting, and Medicare processes, with a schedule Monday–Friday 8a-4p and potential weekend shifts.

Qualified candidates will hold a Nursing Degree and a current RN/LPN license in Massachusetts, with prior MDS experience preferred. Compensation is up to $50 hourly, with competitive benefits.

Qualifications

• Must possess, as a minimum, a Nursing Degree from an accredited school of nursing.
• Must possess a current, unencumbered license to practice as an RN/LPN in Massachusetts.
• Prior experience in MDS preferred.

Responsibilities

• Coordinate the MDS process and resident care plans.
• Oversee ADL training for facility and staff.
• Issue and deliver denial notices timely and appropriately.
• Complete and transmit CMS-approved MDS item sets.
• Follow CMS regulatory guidelines described in the RAI Manual.
• Protect residents' health information and maintain privacy policies.
• Maintain Medicare meeting minutes per program agreement.
• Issue Medicare denial letters per CMS regulations.
• Manage residents' Medicare/Insurance benefits and related documents.
• Perform other duties assigned by manager.
