The Tremont Rehabilitation and Skilled Care Center in Wareham is hiring an MDS Coordinator to lead the MDS process and resident care plan assessments. The role supports CMS compliance, charting, and Medicare processes, with a schedule Monday–Friday 8a-4p and potential weekend shifts.
Qualified candidates will hold a Nursing Degree and a current RN/LPN license in Massachusetts, with prior MDS experience preferred. Compensation is up to $50 hourly, with competitive benefits.
We are hiring an MDS Coordinator to join our dynamic care team at The Tremont Rehabilitation and Skilled Care Center in Wareham!
Shift is Monday-Friday 8a-4p and may include weekends. Can be an LPN or RN. Up to $50 an hour, depending on license and experience.
As the MDS Director, you plan, organize, and direct the MDS process involving overseeing resident care plans through clinical assessment, review of resident's medical history, personal interviews, and completion of MDS reports.
Experience & Education:
Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university.
Must possess a current, unencumbered license to practice as an RN/LPN in this state.
Prior experience in MDS preferred.
Duties & Responsibilities:
Coordination of MDS process
Oversees ADL training for facility and staff.
Issues and delivers denial notices timely and appropriately.
Complete and transmit all CMS approved item sets (MDS)
Must be knowledgeable of and follow current CMS regulatory guidelines as described in RAI Manual.
Complies with facility privacy policies and procedures and protects residents’ individual health information.
Maintains Medicare meeting minutes per Medicare program agreement.
Issues and delivers Medicare denial letters per CMS regulations.
Assures appropriate management of residents’ Medicare/Insurance benefits.
Maintains adequate systems to ensure appropriate documents are sufficient to support billed services.
