Job summary

Caring Acres Nursing and Rehab Center in Anita, Iowa, is seeking a skilled MDS Coordinator to lead individualized care planning, data collection, and resident record accuracy in a supportive care environment.

You will work with a multidisciplinary team to ensure quality care, participate in care conferences and family meetings, and uphold regulatory standards while delivering excellent service to residents and families.

Qualifications

• 2 years of long-term care nursing experience (minimum).
• Education: State of Iowa RN Licensure.

Responsibilities

• Coordinate and implement individualized care plans for residents.
• Gather and analyze resident data to ensure accurate and effective care.
• Collaborate with our multidisciplinary team to provide comprehensive care.
• Participate in care conferences and family meetings.
