Job summary

Lynbrook Restorative Therapy & Nursing is seeking a qualified RN to join as an MDS Coordinator. Urgently hiring with in-person work at our Lynbrook facility in New York. Salary is competitive at $135k per year with comprehensive benefits and 401k matching.

Must be a NYS-licensed RN with strong organizational and communication skills. Experience in sub-acute nursing and rehabilitation is preferred; SAM/SIGMACARE experience is a plus.

Qualifications

• Must be a NYS Licensed RN.
• Detail-oriented and highly organized.
• Able to work independently and as part of a team.
• Strong communication skills and deadlines-driven PPS/MDS knowledge.

Responsibilities

• Coordinate MDS assessments and PPS submissions.
• Ensure PPS/MDS process compliance and accuracy.
• Meet scheduling deadlines and collaborate with care teams.

Previous MDS experience required. Sub-acute nursing and rehabilitation experience preferred.

Requirements:

NYS Licensed RN, must be a detail-oriented, highly organized, dedicated professional who is able to work independently and as part of a team.

• Excellent communication skills a must, with the ability to follow oral and written instruction.
• Solid computer skills required, SIGMACARE experience preferred.
• Has a working knowledge of MDS and PPS process and able to meet deadlines.
• Ability to work hours as scheduled based on the requirements of the position assignments.
