Sutton Park Center for Nursing and Rehabilitation in New Rochelle, NY is seeking a qualified RN to join our team as an MDS Coordinator. This full-time role focuses on MDS and PPS processes within a sub-acute nursing and rehabilitation setting, with CPT approval and an urgent need to fill the position.

You will work in person, coordinate assessments, meet deadlines, and collaborate with the care team to ensure accurate documentation and compliant care.

Qualifications

• Must hold NYS RN license and be a detail-oriented, organized professional.
• Ability to work independently and as part of a team.
• Excellent written and verbal communication skills.
• Solid understanding of MDS and PPS processes with deadlines.

Responsabilités

• Coordinate MDS assessments and PPS workflows.
• Ensure timely submission and accuracy of documentation.
• Collaborate with medical directors and care teams.
• Maintain compliance with regulations and company policies.

Previous MDS experience required. Sub-acute nursing and rehabilitation experience preferred.

Requirements:

NYS Licensed RN, must be a detail-oriented, highly organized, dedicated professional who is able to work independently and as part of a team.

• Excellent communication skills a must, with the ability to follow oral and written instruction.
• Solid computer skills required, SIGMACARE experience preferred.
• Has a working knowledge of MDS and PPS process and able to meet deadlines.
• Ability to work hours as scheduled based on the requirements of the position assignments.

Job Type: Full-time

Salary:up to $140k per year
