Job summary

Waters Of Indianapolis is seeking an experienced MDS Coordinator to join our long-term care team. The RN will conduct federally mandated assessments and compile data to guide care planning and reimbursement under RUG-IV guidelines.

The role requires coordination of OBRA schedules, MDS completions, and communication with residents, families, and interdisciplinary teams to ensure timely and accurate documentation.

Qualifications

• RN license required.
• MDS certification (AANAC) optional.

Responsibilities

• The MDS Coordinator reports to the facility Administrator.
• Complete OBRA, PPS and Managed Care MDS.
• Prepare Admit MDS Tracking Forms, Death in the Facility Tracking Forms, and Discharge Assessments per the RAI Manual.
• Coordinate Care Plans with other disciplines to ensure timely initiation or revision.
• Convene Care Plan Conferences within 21 days of admission and every 90 days thereafter.
• Prepare OBRA/PPS/MDS communications and documentation per CMS guidelines.
• Maintain and review MDS-related documentation to support billing and care planning.
