Qualifications

• Experience as MDS Coordinator in a Skilled Nursing Facility.

• Understanding of Medicare, Medicaid, and SNF reimbursement requirements.

Responsibilities

• Coordinate and oversee the MDS assessment process.
• Ensure assessments are accurate, timely, and CMS-compliant.
• Collaborate with Nursing, Therapy, Social Services, Dietary and other team members.
• Lead and facilitate care plan development and coordination.
• Monitor documentation and identify opportunities to improve outcomes.
• Support regulatory compliance and survey readiness.
• Maintain certs/recerts and initiate death certificates.
