Job summary

Winchester Rehabilitation and Nursing Center in Winchester, MA is seeking an MDS Nurse RN/LPN to join our 5-star facility and provide high-quality care to our residents alongside a dedicated, compassionate team.

You will conduct and coordinate resident assessments, implement care plans with the interdisciplinary team, ensure PPS/MDS compliance, and contribute to a warm, welcoming environment for our patients.

Qualifications

• Current unencumbered State of Massachusetts RN or LPN License
• Two years of MDS and PPS experience
• Care Planning experience

Responsibilities

• Conduct and coordinate the development and completion of the resident assessment
• Conduct resident assessments in accordance with current rules, regulations, and guidelines
• Implement resident CAA's
• Participate in facility survey readiness and throughout the survey
• Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
