Job summary

RegalCare at Glen Ridge is seeking an experienced MDS Coordinator for a 40 per hour position in a facility-based setting. The role focuses on accurate MDS assessments, RAI management, and collaborative care planning to support quality outcomes and reimbursement.

Candidate must hold an active RN or LPN license and have prior MDS experience in long-term care. Strong knowledge of Medicare/Medicaid guidelines and RAI requirements is required, with excellent organizational skills and ability to meet

Responsibilities

• Coordinate and complete accurate, timely MDS assessments in accordance with state and federal regulations.
• Manage the RAI process and support interdisciplinary care planning.
• Ensure documentation supports quality outcomes and reimbursement.
• Collaborate with nursing, therapy, and administrative teams.
• Assist with audits, surveys, and quality improvement initiatives.
