Job summary

West Ridge Specialty Care in Knoxville, IA is seeking an experienced MDS Coordinator to lead accurate assessments, care planning, and regulatory compliance for our 78-bed long-term care community.

You will collaborate with an interdisciplinary team, support audit readiness, and monitor documentation to ensure quality and proper reimbursement, while guiding nursing staff on MDS standards and resident communication.

Qualifications

• RN license valid in Iowa and in good standing.
• Current CPR certification.
• Knowledge of federal and state long-term care regulations.
• Desire to learn and grow within a quality-focused clinical team.
• Strong empathy, respect, patience and discretion.

Responsibilities

• Complete and coordinate accurate, timely MDS assessments to evaluate resident needs.
• Collaborate with interdisciplinary teams to develop and update individualized care plans.
• Ensure CMS and state/federal long-term care regulatory compliance.
• Monitor and validate clinical documentation for accurate reimbursement and quality.
• Analyze resident data to support care quality and clinical decision-making.
• Serve as resource on MDS processes and documentation standards.
• Participate in care conferences and communicate with residents and families.
• Support audit readiness and maintain survey-ready documentation.
• Enforce policies and guide the team, promoting growth.
• Communicate with residents and families about care needs.
• Document care to meet clinical and facility standards.
• Follow infection control, safety, and compliance protocols.
• Participate in on-call rotation with leadership team.

What You’ll Do & Key Responsibilities:

• Communicate effectively with residents and families regarding care needs.
• Document care accurately to meet clinical, regulatory, and facility standards.
• Follow infection control, safety, and compliance protocols at all times.
• Participate in an on-call rotation with other members of the leadership team.

Qualifications:

• Valid RN license in good standing. License must be valid within the state of Iowa, or ability to activate.
• Current, valid CPR Certification.
• Knowledge of federal and state long-term care regulations and laws.
• A desire to learn and grow as part of a quality focused clinical team.
