As an RN MDS Coordinator, you will complete and ensure the accuracy of Minimum Data Set (MDS) assessments for all residents. You will contribute to personalized resident care plans and ensure the capture of clinical reimbursement for services provided. You will manage short-term managed care and commercial cases.
Your responsibilities include gathering information, assessing needs, establishing goals, and providing interventions within an organized, concise, functional care plan. You will coordinate the completion of comprehensive MDS assessments by the interdisciplinary team and incorporate recommendations into individual resident care plans. These plans must identify all relevant issues and goals for each resident. You will work with the care planning team to implement final care plans and encourage resident and family participation in developing and reviewing care plans, focusing on helping residents reach their highest practicable level of well-being.
You will ensure that all nursing personnel are aware of the care plans and that they are used in daily nursing services. You will review nurses’ notes, monitor residents to ensure care plan adherence, and revise care plans as needed. Additionally, you will plan, schedule, and conduct weekly care plan meetings for all residents in accordance with OBRA and state requirements.
