Company Description:

Mystic Healthcare is a 100-bed, family-owned skilled nursing facility in Mystic, Connecticut, specializing in short-term rehabilitation and long-term care. We are currently hiring an MDS Coordinator (RN). Come join our team!

Responsibilities:
  1. Conduct and coordinate the development and completion of the resident assessment (MDS) according to current rules, regulations, and guidelines, including RAPs and Triggers. Electronically transmit all data to the State following HCFA and Connecticut laws.
  2. Develop preliminary and comprehensive MDS assessments of residents' nursing needs, utilizing required forms and facility policies.
  3. Ensure each assessment section is signed and dated by the responsible staff.
  4. Adhere to MDS assessment schedules for sub-acute and Medicare patients.
  5. Electronically submit MDS data to the State of Connecticut at least every 30 days, as required.

Requirements:
  1. Possess a Nursing Degree from an accredited college or university and be a graduate of a Connecticut-approved RN program.
  2. Hold a current certification as a Nurse Aide.
  3. Ability to communicate effectively in English, both written and spoken.
  4. Physical ability to push, pull, move, and/or lift at least 40 pounds, and to carry such weight for at least 4 feet.
  5. Proof of Covid-19 vaccination and booster.
