Job description

Berkshire Nursing & Rehabilitation Center is committed to improving our patients' quality of life & autonomy through innovative rehabilitative services and an exceptional level of client-centered care and attention.

We are seeking an MDS Coordinator to join our interdisciplinary team of skilled health care professionals at our skilled nursing facility in West Babylon!

This is an excellent opportunity for an RN looking to move into a nonclinical role.

Job responsibilities include but are not limited to:

  1. Completing accurate assessments, MDS & care plans as assigned.
  2. Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDS coding.
  3. Initiating care plans and supporting activities as assigned.
  4. Maintaining & updating all care plans and assessments as required.
  5. Monitoring & auditing clinical records, ensuring accuracy & timeliness.
  6. Protecting the confidentiality of Resident & Facility information at all times.
  7. Monitoring & auditing clinical records, ensuring accuracy & timeliness.

REQUIREMENTS:

  1. Experience with MDS 3.0 preferred.
  2. Valid NY State RN License.
  3. Long Term Care experience preferred.
  4. Must be highly organized, professional & eager to learn.
  5. Should have solid computer skills.
  6. Excellent communication skills.
