Job summary

Topaz HR Advisory is seeking an experienced MDS Coordinator for a Brooklyn, NY-based skilled nursing facility. The role oversees the MDS process, ensures accurate clinical assessments, and supports reimbursement optimization while ensuring regulatory compliance.

The RN-licensed candidate will collaborate with interdisciplinary teams, maintain documentation accuracy, participate in audits, and contribute to quality resident care and operational performance.

Qualifications

• Active RN license required.
• Experience with MDS in skilled nursing or long-term care.
• Strong understanding of Medicare, Medicaid, CMS regulations and reimbursement processes.
• Excellent organizational, communication and documentation skills.
• Ability to multitask and meet deadlines.

Responsibilities

• Coordinate and complete MDS assessments in accordance with state and federal regulations.
• Ensure timely and accurate MDS submissions.
• Collaborate with nursing, therapy, and interdisciplinary teams on care planning.
• Support Medicare and Medicaid reimbursement optimization.
• Monitor documentation accuracy and compliance standards.
• Participate in audits, surveys, and quality improvement initiatives.
• Maintain current knowledge of CMS guidelines and reimbursement practices.
• Assist with care plan development and resident assessment coordination.
• Ensure accurate clinical documentation to support reimbursement and compliance objectives.
• Communicate effectively with facility leadership and clinical staff regarding assessment updates and requirements.

Position Overview
We are seeking an experienced MDS Coordinator to oversee the MDS process, ensureaccurateclinical assessments, and support reimbursement optimization within the facility. • Ability to multitask and manage deadlines effectively
• Strong attention to detail and regulatory compliance standards
• Ability to collaborate effectively within interdisciplinary healthcare teams

Compensation
The published salary range is used as a guide to provide prospective candidates with a level of compensation that is competitive with today’s market. The salary range for this position is a good faith estimate that allows for flexibility to align with various levels of experience, education, and performance.
