Skylake Post Acute in Thornton, Colorado, seeks an experienced full-time MDS Coordinator (RN/LPN) to join our skilled nursing facility team. You will handle MDS documentation, ensure timely data processing, and maintain chart integrity in a busy SNF setting.

Qualified candidates hold an RN or LPN license with MDS 3.0 experience and prior SNF exposure. We offer competitive pay, benefits, and a supportive work environment.

What to Expect:

• Collects MDS documents within mandated, established and expected time frames.
• Compares data entered against the original MDS document; makes corrections as needed.
• Files the MDS documents in residents' charts.
• Maintains confidentiality of all patient care information.

Successful Candidates:

• Current RN/LPN License
• Experience as a MDS Coordinator utilizing 3.0
• Experience in a SNF environment

Rate Range: LPN $35-$40 RN $41-$50/ hour
