Job summary

Luxor Healthcare Management is hiring an experienced MDS Coordinator to lead the MDS assessment process and ensure accuracy in documentation for residents in our Tennessee facilities.

In this role you will collaborate with nursing, therapy, dietary, social services, and other disciplines, participate in care plan meetings, monitor regulatory compliance, and support timely coding and reporting to reflect resident needs and services.

Qualifications

• Previous MDS experience in a skilled nursing or long-term care setting.
• Strong knowledge of the MDS 3.0 process and CMS regulations.
• Excellent organizational and time-management skills.
• Strong attention to detail.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Strong communication and documentation skills.
• Current nursing license or appropriate clinical credentials, as required.

Responsibilities

• Coordinate and complete the MDS assessment process in accordance with CMS and regulatory requirements.
• Ensure accurate and timely completion of MDS assessments and related documentation.
