Job summary

River Valley Rehabilitation & Healthcare Center in Del Norte, CO, is seeking a qualified MDS/Care Plan Coordinator (LPN/RN) to lead the facility's Medicare/Medicaid programs and ensure residents receive high-quality, individualized care.

This role focuses on coordinating MDS, PPS, OBRA assessments, preparing care plans, and collaborating with nursing, therapy, physicians, residents, and families. A Colorado nursing license is required, with LTC experience preferred.

Qualifications

• Licensed LPN or RN in Colorado.
• Graduate of an accredited nursing program.
• 2+ years long-term care experience preferred.
• Knowledge of RAI, Medicare, Medicaid, and PPS processes.
• Strong communication, organization, and leadership skills.
• Detail-oriented, dependable, and team-focused.

Responsibilities

• Coordinate and manage MDS, PPS, and OBRA assessment schedules.
• Complete accurate and timely resident assessments and care plans.
• Monitor Medicare and Medicaid eligibility and coverage requirements.
• Conduct pre-admission nursing screenings as assigned.
• Coordinate interdisciplinary Medicare meetings and care plan conferences.
• Ensure timely submission of documentation and regulatory reports.
• Collaborate with nursing staff, therapy teams, physicians, residents, and families.
• Monitor resident condition changes and update care plans as needed.
• Ensure compliance with federal, state, and facility regulations.
• Support quality assurance initiatives and reimbursement accuracy.
