GLENDALE GARDENS NURSING & REHAB in Springfield, Missouri is seeking an MDS Coordinator to join our team. This role focuses on completing accurate MDS assessments and care plans, and initiating coordinating activities for residents.

Ideal candidates hold a valid RN or LPN license with prior MDS experience in long-term care. The position requires a 40-hour work week, strong organization, professional demeanor, and excellent communication within a collaborative team.

Qualifications

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records for accuracy and timeliness
• Informing DON of non-compliant documentation issues
• Protecting confidentiality of Resident & Facility information
• Participating in meetings and committees (Medicare, QAPI, Weight Management, etc.)

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

• Valid RN or LPN License
• Knowledge of skilled nursing facilities survey process and regulations would be helpful.
• Must have prior MDS Experience
• Long Term Care Experience Required
• Must be able to work 40 hours a week
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker
