Potosi Manor in Potosi, MO is seeking an MDS Coordinator to join our team. The role emphasizes completing MDS assessments, care plans, and maintaining calendars to ensure timely and accurate documentation.

Ideal candidates hold a valid RN or LPN license with prior MDS experience in long-term care; must be highly organized with strong communication and computer skills. This is a full-time, on-site position with opportunities to contribute to QAPI initiatives and care quality.

Qualifications

• Valid RN or LPN license is required.
• Prior MDS experience in long-term care preferred.
• Knowledge of skilled nursing facility survey process helpful.
• Must be able to work 40 hours per week.
• Highly organized, professional and motivated.
• Solid computer skills and excellent communication.
• Friendly, team-oriented approach.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.

• Monitoring & auditing clinical records for accuracy and timeliness.

• Participates in meetings and committees such as Medicare, QAPI, Weight Management.

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Maintaining & updating all care plans and assessments as required

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Informing DON of persistent issues related to non-compliant documentation

• Protecting the confidentiality of Resident & Facility information at all times

• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

• Valid RN or LPN License
