Job Summary

The MDS/Care Plan Coordinator is an experienced healthcare provider responsible for ensuring accurate assessments and up-to-date care plans for residents. They ensure all records related to resident care are consistent with the care plan and accurately describe the care provided. The CPC communicates resident conditions to families, coordinates comprehensive assessments, and reports to the Administrator.

Qualifications
  • A current State of Illinois license as a Registered Nurse or Licensed Practical Nurse.
  • Ability to read, write, and speak understandable English.
  • AANAC certification preferred.
Job Requirements
  • Physical and mental capability to perform routine duties.
  • Knowledge of nursing services, administration, rehabilitation, geriatric nursing, the MDS, and EMR systems.
  • Strong managerial, organizational, planning, computer, and interpersonal skills.
  • Ability to initiate, complete, and update care plans.
  • Ability to train staff on care plans, documentation, and EMR usage.
  • Monitor EMR records for consistency and content.
  • Identify and communicate problems to the Director of Nursing and staff.
  • Maintain professionalism and compassion with residents, families, and staff.
  • Knowledge of JCAHO, OBRA, IDPH, and HFS standards and documentation methods.
Main Duties
  • Support the facility's philosophy of care and strive to meet its goals.
  • Coordinate and ensure timely completion of MDS assessments upon admission, readmission, quarterly, annually, or after significant changes.
  • Schedule and oversee MDS assessments and Care Plan Conferences.
  • Review diagnosis coding with physicians quarterly and update ICD-9 codes as needed.
  • Conduct weekly MDS pre-planning and Medicare meetings.
  • Assist staff with EMR charting and documentation to meet care plan goals.
  • Communicate care plans and progress to staff, residents, and families.
  • Train nursing staff periodically on documentation and care plans.
  • Audit EMR charting for accuracy and address discrepancies.
  • Collaborate with other departments to ensure consistent care.
  • Coordinate with therapy and dietary departments for comprehensive resident care.
  • Maintain confidentiality and adhere to safety, infection control, and emergency policies.
