MDS Coordinator - Nursing Home

Location: Oak Lawn, IL

Salary: Up to $90K per year (Depending on Experience)

A beautiful, privately owned skilled nursing facility located in Oak Lawn, IL is seeking an experienced and knowledgeable MDS Coordinator.

Responsibilities of the MDS Coordinator:

1. Ensure utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
2. Observe direct nursing care, review documentation, and make appropriate recommendations. Assist with chart audits.
3. Provide clinical support and direct staff to appropriate resource materials.

Requirements:

1. Must have long-term care experience.
2. Must have experience as an MDS Coordinator.
3. Must hold an Illinois RN license.
4. Must be familiar with MDS 3.0.
