Position Summary:

The MDS Coordinator is responsible for coordinating and ensuring the completion of all resident scheduled MDSs, CAAs, and Care Plans. They manage MDS functions to maximize clinical outcomes, ensure timely and accurate documentation, and enable full reimbursement from government payers. The role requires fluency in scheduling, coding, and organizational skills.

Qualifications:

• Current valid Maine nursing license
• At least one year of MDS experience in long-term care
• Leadership, organizational skills, and a positive attitude
• Effective communication skills in speech and writing
• Up-to-date physical assessment skills and comprehensive nursing knowledge

Essential Functions:

• Participate in inter-disciplinary care plan and Medicare team meetings
• Develop and maintain resident care plans
• Coordinate resident and responsible party participation in care meetings
• Review therapy participation and assessment schedules
• Inform IDT of significant resident condition changes
• Coordinate MDS assessments according to schedule
• Perform or ensure nursing staff perform MDS assessments timely
• Complete additional MDS sections if team members are absent
• Monitor compliance in MDS and care planning; report issues
• Lead weekly Utilization Review/Medicare meetings
• Complete and submit billing logs
• Conduct stand-up meetings with IDT and rehab teams
• Knowledge of RUGS and PDPM
