Overview

Part Time MDS Coordinator Needed - 20 Hours/Week

Responsibilities
  1. Ensure appropriate reimbursement for patient care services by participating in the Patient Assessment Process, including completion of MDS and Care Area Assessments (CAAs).
  2. Assist in developing individualized patient care plans, identifying patient needs, and projecting outcomes as required by Federal and State regulations.
  3. Contribute to the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring proper completion, encoding, and electronic transmission of the MDS.
  4. Identify changes in patient condition, coordinate assessments, and collaborate with the interdisciplinary team.
  5. Assist with Activities of Daily Living training for new hires and provide ongoing education for existing staff.
  6. Attend educational programs to stay current with RAI and Prospective Payment System regulations.
Qualifications
  • Must be a Registered Nurse licensed by the state and in good standing.
  • Experience in a hospital, long-term care facility, or related healthcare setting is required.
  • Prior experience with completing MDSs and Care Plans is required.
  • Knowledge of general, rehabilitative, and restorative nursing practices, laws, regulations, and guidelines governing long-term care.
