Job summary

Abernethy-Laurels in Newton, NC is seeking a qualified Registered Nurse to join our Christian ministry focused on vibrant living for residents in a long-term care setting. This full-time role requires active RN licensure, a nursing degree, and at least one year of geriatric experience.

  • Strong organizational, communication, and analytical skills.

Responsibilities

  • Coordinate interdisciplinary team assessments and resident care planning processes.
  • Complete, review, and audit MDS assessments for accuracy and timeliness.
  • Identify and calculate Resident Assessment Protocol (RAP) triggers.
  • Interview residents and resident representatives and document findings.
  • Conduct weekly healthcare planning meetings.
  • Monitor resident status changes and ensure appropriate assessment updates are completed.
  • Review and update resident care plans in collaboration with residents, families, and the interdisciplinary team.
  • Participate in admission and discharge planning.
  • Analyze quality assurance data and report findings to leadership and the Quality Assessment and Assurance Committee.
  • Collaborate with department leaders to develop and implement quality improvement initiatives.
  • Provide staff education and training related to assessment and care planning processes.
