Job summary

Park Nursing Home in Rockaway Park, NY is seeking an experienced RN MDS Nurse / Coordinator for our Skilled Nursing Facility. You will oversee MDS assessments, care plans, and documentation to ensure quality resident care.

The ideal candidate holds a NYS RN license, is highly organized, tech-savvy, and a strong communicator who works well in a team and with facility staff. This role is on-site at our Rockaway Park facility.

• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
