Qualifications

  • Current RN license in the applicable state.
  • Previous long-term care or skilled nursing experience is required.
  • Strong organizational, time management, and critical thinking skills.
  • Proficient with electronic medical records and computer applications.
  • Excellent verbal and written communication skills.
  • Ability to work collaboratively as part of an interdisciplinary team while managing multiple priorities.

Responsibilities

  • Complete accurate MDS assessments and comprehensive care plans in accordance with regulatory requirements.
  • Initiate and update resident care plans while collaborating with the interdisciplinary care team.
  • Prepare and distribute monthly care plan schedules in a timely manner.
  • Maintain and update resident assessments and care plans as required.
  • Monitor and audit clinical documentation to ensure accuracy, completeness, and compliance.
  • Identify documentation concerns and communicate ongoing compliance issues to nursing leadership.
  • Maintain strict confidentiality of resident, employee, and organizational information.
  • Support quality assurance initiatives and contribute to positive survey outcomes.

A well-established long-term care organization is seeking an experienced MDS Coordinator (RN) to join our clinical leadership team. The ideal candidate is detail-oriented, organized, and knowledgeable in MDS processes and regulatory requirements. This position plays a key role in ensuring accurate clinical documentation, reimbursement, and quality resident care.


Key Responsibilities



  • Complete accurate MDS assessments and comprehensive care plans in accordance with regulatory requirements.

  • Initiate and update resident care plans while collaborating with the interdisciplinary care team.
