Job summary

Park Nursing Home in Rockaway Park, NY, a 196-bed Skilled Nursing Facility by Rockaway Beach, is hiring an MDS Assessor to work in-person.

Required: Registered Nurse (RN) license in New York; MDS experience preferred; long-term care experience. You will complete assessments and care plans, maintain records, and work as part of a professional team.

Responsibilities

• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records, ensuring accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident and Facility information at all times.

Park Nursing Home is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Rockaway Park, NY. We are a 196 bed facility located right by the boardwalk at Rockaway Beach. We have a beautiful view of the Atlantic Ocean and we are accessible to the subway, water ferry and buses.

Required: Registered Nurse (RN) License

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required

REQUIREMENTS
