Job description

We are looking for a strong individual to fill our MDS Coordinator position.

RN licensure within the state plus a minimum of two years MDS experience in long term care preferred. LVN or LPN may be considered with exceptional qualifications; or an equivalent combination of education and experience.

Position Purpose:

Responsible for the timely and accurate completion of the MDS process from admission to discharge. May coordinate information systems, operations, and education for the clinical department.

Essential Functions of the Position:

• Works in collaboration with the facility team to assess the needs of the resident.
• Assists with the coordination and management of care management meetings, including review of residents' care and setting assessment reference dates.
• Complies with federal and state regulations regarding completion and coordination of the MDS process.
• Monitors MDS and care plan documentation for all residents.
• Ensures documentation is present in the medical record to support MDS coding.
• Maintains current MDS status of assigned residents according to state and federal guidelines.
• Supports the tracking system of MDS schedules (timeframes and due dates).
• Maintains accurate data entry of residents' information into appropriate computerized programs.
• Completes accurate coding of the MDS with information obtained via medical record review, observation, and interviews with facility staff, residents, and family members.
• Attends team meetings and other meetings as required to gather information, communicate changes, and update records.
• Continuously updates knowledge related to data entry and computer technology.
• Participates in quality assurance activities as required.
• Completes electronic submission of required documentation to relevant databases and entities per company policy.
• Performs additional duties as assigned.
