The Remote MDS Coordinator will collaborate with contract partners to plan, organize, and coordinate the completion of the Minimum Data Set (MDS) in accordance with Federal and State Regulations. Key Responsibilities:

• Schedule and coordinate MDS assessments to ensure timeliness per RAI guidelines, including care plan development with the interdisciplinary team
• Oversee Medicare, Medicaid, and Managed Care to ensure appropriate clinical services and reimbursement
• Develop and review individualized resident care plans, updating quarterly
• Verify MDS documentation and charting to support clinical services
• Ensure timely submission of MDS data to the state database with appropriate follow-up
• Utilize QI/QM data to promote quality care, identify trends, and improve processes

Qualifications:

• 3+ years of nursing experience in MDS assessment
• RAC-CT preferred
• RN or LPN license in Iowa
• Reside in Iowa
• Thorough understanding of PDPM requirements
• Strong computer skills and ability to navigate EMR
• Excellent customer service skills
