Job summary

Accela Rehab and Care Center at Manalapan is seeking an experienced MDS Coordinator to join our on-site team. You will conduct comprehensive resident assessments, develop individualized care plans, and ensure timely submission of MDS assessments while maintaining regulatory compliance.

The ideal candidate is a licensed RN or LPN in New Jersey with prior MDS coordination experience in long-term or short-term care settings, and a collaborative approach to improve resident outcomes and care

Qualifications

• Licensed RN or LPN in New Jersey.
• Experience in MDS coordination in long-term or short-term care settings.

Responsibilities

• Conduct comprehensive resident assessments and develop individualized care plans.
• Accurately complete and submit MDS assessments within regulatory timeframes.
• Collaborate with interdisciplinary teams to enhance quality care and resident outcomes.
• Monitor and maintain compliance with MDS regulations and guidelines.
• Support facility initiatives for continuous improvement and resident satisfaction.
• Support residents with daily activities, promoting comfort and safety.
