Accela Rehab and Care Center at Manalapan is seeking an MDS Coordinator to join our nursing team in Englishtown, NJ. The ideal candidate is a licensed RN or LPN with MDS coordination experience and a passion for resident-centered care.

You will plan, conduct, and submit MDS assessments on time, collaborate with interdisciplinary teams, and help ensure regulatory compliance and continuous quality improvement while supporting residents with daily activities.

Qualifications

• Licensed RN or LPN in New Jersey.
• Previous experience coordinating MDS in long-term or skilled care settings.
• Knowledge of MDS regulations and submission timelines.

Responsibilities

• Conduct comprehensive resident assessments and develop individualized care plans.
• Accurately complete and submit MDS assessments within regulatory timeframes.
• Collaborate with interdisciplinary teams to enhance quality care and resident outcomes.
• Monitor and maintain compliance with MDS regulations and guidelines.
• Support facility initiatives for continuous improvement and resident satisfaction.
• Assist residents with daily activities, promoting comfort and safety.
