Job summary

Cedars of Lebanon is seeking an experienced MDS Coordinator to manage the RAI process, ensure timely MDS submissions, and support care planning for long-term residents.

You will oversee assessments, coordinate care conferences, and ensure compliance with federal and state regulations, leveraging your RN or LPN background and CPR certification. This full-time role requires a minimum of 3 years in long-term care as an MDS Coordinator.

Qualifications

• Must hold a Nursing Degree from an accredited college or university.
• Current, unencumbered, active RN license or LPN and CPR.
• At least 3 years' experience in Long Term Care as a MDS Coordinator.
• RAC-CT Preferred.

Responsibilities

• Monitor and guide the completion of assessments in a timely manner.
• Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtaining receipt of validation.
• Schedule and facilitate care plan conferences and monthly QM meetings.
• Manage Care Planning Process.

We are seeking an experienced full-time MDS Coordinator to join our dedicated team at Cedars of Lebanon, As an MDS Coordinator, you will play a crucial role in coordinating the RAI process, ensuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.

Mandatory Qualifications
