Job summary

Fusion Healthcare Rehabilitation and Nursing Center in Westfield, MA is seeking a Per Diem RN to join our team. The role requires prior MDS experience in long-term care, strong organization, and excellent communication skills.

Qualifications

Responsibilities

• Complete accurate assessments, MDS and care plans as assigned.
• Initiate and support care plans and activities.
• Create and distribute monthly care plan calendars promptly.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform DON of persistent documentation issues.
• Protect resident and facility confidentiality.

Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

Requirements:

• Valid RN License
• MUST HAVE PRIOR MDS EXPERIENCE
• Long Term Care Experience Required
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker

Experience:

• MDS: 2 years (Required)
• long term care: 2 years (Required)

License:
