MDS / Care Plan Coordinator - RN or LPN

Location: Rancho Rehab & Healthcare - Florissant, MO

Job Type: Full-Time

Rancho Rehab & Healthcare is a 120-bed skilled nursing and rehabilitation facility in Florissant, MO, dedicated to clinical excellence and compassionate care. We are currently seeking an experienced and detail-oriented MDS/Care Plan Coordinator (RN or LPN) to join our dynamic team!

Key Responsibilities:

• Coordinate and oversee the MDS assessment and care planning process to ensure accuracy and compliance with RAI guidelines, Medicare/Medicaid, and state regulations
• Develop and update individualized plans of care for residents based on assessments and changing needs
• Schedule and manage all resident assessments and documentation for Medicare, Medicaid, and Managed Care reimbursement
• Participate in the pre-admission process and determine level of care upon admission
• Monitor and analyze Quality Indicator Reports to identify concerns and ensure care plans reflect accurate information
• Ensure timely completion and submission of all MDS assessments, certifying documentation as required
• Stay up-to-date with state and federal guidelines related to MDS, PPS (Prospective Payment System), and care planning
• Collaborate with interdisciplinary team members to maintain compliance and support best resident outcomes
  
  
  

Qualifications:

• Current RN or LPN license in the state of Missouri (Required)
• Prior experience as an MDS Coordinator (Required)
• In-depth knowledge of MDS 3.0, RAI process, care planning, and PPS
• Strong organizational and time management skills
• Ability to lead, communicate, and collaborate with interdisciplinary teams
• Familiarity with electronic medical records (EMR) systems
