Position Summary

The MDS Coordinator manages the completion of the Minimum Data Set (MDS) with the interdisciplinary team, facilitating assessments and ensuring timely transmission according to regulatory standards.

Essential Duties and Responsibilities

• Schedule ARDs for Payment Assessments and coordinate with the IDT for assessment completion
• Ensure assessments are completed, transmitted, and validated within specified timeframes
• Maintain validation reports and facilitate corrections as needed
• Participate in staff education and quality improvement activities
• Compile data reports and utilize facility software to meet federal requirements
• Assist in care plan updates and identify residents needing Significant Change in Status Assessments
• Adhere to HIPAA policies and promote a culture of high performance and collaboration

Physical Demands and Work Environment

This role involves regular walking, contact with contagious illnesses, and adherence to no-smoking policies. The position operates 24/7, including weekends and holidays.

Qualifications

• Graduate of an accredited School of Nursing
• Minimum 1 year healthcare experience as a licensed nurse or MDS Coordinator
• Valid Texas LPN/RN license and CPR certification

Preferred experience includes PPS system knowledge, 2+ years as a licensed nurse, and 2+ years in an MDS role.
