• Graduate of an approved RN / LVN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.

Job Summary

Forest Parking Nursing Center is looking for an experienced and friendly MDS Coordinator to join our team. The MDS Coordinator will be responsible for timely and accurate completion of the RAI process and care management from admission to discharge, in accordance with company policies, federal, state, and certification guidelines. They may also coordinate information systems operations and education for the clinical department.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required.
• Ensure decisions on completing MDS assessments are based on clinical criteria per the latest RAI User’s Manual.
• Coordinate and manage daily stand-up meetings, review resident care, and set assessment reference dates.
• Comply with federal and state regulations regarding the MDS process.
• Monitor MDS and care plan documentation for all residents, ensuring proper documentation supports MDS coding.
• Maintain current MDS status for residents according to guidelines.
• Accurately enter resident data into computerized MDS programs.
• Code MDS accurately based on medical records, observations, and interviews.

Qualifications

• Graduate of an approved RN / LVN program and licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA regulations, and MDS 3.0 is preferred.
• Understanding of the care planning process.
