Job summary

Heart Healthcare Management PA LLC seeks an experienced MDS Coordinator to oversee the RAI process, ensure timely MDS assessments, and support regulatory compliance. The role requires an RN licensed in PA with at least 2 years in skilled nursing or long-term care, plus strong knowledge of RAI, PDPM, and CMS guidelines.

You will collaborate with an interdisciplinary team and participate in care plan meetings to ensure high-quality resident care.

Qualifications

• RN license in PA is required.
• Minimum of 2 years of experience in a skilled nursing or long-term care setting.
• Prior MDS Coordinator experience is required.
• Strong knowledge of RAI, PDPM, and CMS guidelines.
• Excellent organizational, analytical, and communication skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state guidelines.
• Ensure timely and accurate submission of MDS assessments to regulatory agencies.
• Develop, review, and update resident care plans based on assessment findings.
• Collaborate with interdisciplinary team members to ensure quality resident care.
• Monitor and maintain compliance with CMS regulations and Pennsylvania State Requirements.
• Participate in care plan meetings with residents and families as needed.
• Conduct audits to ensure documentation accuracy and regulatory compliance.
• Provide education and guidance to staff regarding MDS processes and documentation.

We are seeking an experienced and detail-oriented MDS Coordinator to join our skilled nursing and long-term care team. The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, and supporting compliance with federal and Pennsylvania state regulations.

Key Responsibilities:

• Coordinate and complete Minimum Data Set (MDS) assessments in accordance with federal and state guidelines
• Ensure timely and accurate submission of MDS assessments to regulatory agencies
