Hallmark Healthcare of Carlinville, in Carlinville, IL, is seeking a compassionate MDS Care Plan Coordinator (RN/LPN) to join our growing team. You will oversee resident assessments, complete MDS, and develop individualized care plans to ensure residents receive high-quality care.

The role requires an Illinois RN or LPN license, long-term care experience, and 1–2 years of MDS experience. Strong communication, organization, and documentation skills are essential.

Qualifications

• Current Illinois RN or LPN license required.
• Long-term care experience required.
• 1–2 years of MDS experience preferred.
• Strong understanding of long-term care nursing practices and regulations.
• Excellent communication, organizational, and documentation skills.
• Dependable and professional team player with reliable attendance.
• Passionate about providing compassionate care and improving resident outcomes.

Responsibilities

• Complete and maintain accurate MDS assessments and documentation.
• Develop, review, and revise individualized resident care plans.
• Ensure accurate documentation for reimbursement from all payor sources.
• Coordinate with interdisciplinary team members to support quality resident care.
• Monitor resident progress and update assessments as needed.
• Support compliance with long-term care regulations and facility policies.
• Help maximize resident outcomes and appropriate resource utilization.
• Maintain accurate and timely documentation related to assessments and care planning.
• Support resident dignity, safety, and well-being.
• Perform additional duties as assigned by management in a resident-centered care environment.
