Responsibilities

• Complete and maintain accurate MDS assessments and documentation
• Develop, review, and revise individualized resident care plans
• Ensure accurate documentation for reimbursement from all payor sources
• Coordinate with interdisciplinary team members to support quality resident care
• Monitor resident progress and update assessments as needed
• Support compliance with long-term care regulations and facility policies
• Help maximize resident outcomes and appropriate resource utilization
• Maintain accurate and timely documentation related to assessments and care planning
• Support resident dignity, safety, and well-being
• Perform additional duties as assigned by management in a resident-centered care environment

Your Role:

Who We’re Looking For:

• Current Illinois RN or LPN license required
• Long-term care experience required
• 1–2 years of MDS experience preferred
• Strong understanding of long-term care nursing practices and regulations
• Excellent communication, organizational, and documentation skills
• Dependable and professional team player with reliable attendance
• Passionate about providing compassionate care and improving resident outcomes
