Job summary

The Pavilion at Edgefield in Canton, Ohio is seeking an experienced MDS Coordinator to join our clinical leadership team. This is a hands-on role focused on accurate, timely MDS assessments and PDPM reimbursement capture, while supporting care planning across disciplines.

Candidates must hold an active Ohio RN or LPN license and have prior MDS experience in a SNF/LTC setting, with strong collaboration across nursing, therapy, and other professionals to ensure survey readiness.

Qualifications

• Active RN or LPN license in Ohio.
• Prior MDS Coordinator or MDS Nurse experience required.
• Experience in a SNF/LTC setting is required.

Responsibilities

• Coordinate and complete MDS assessments accurately and on schedule.
• Manage the RAI process, including care plan coordination.
