Job summary

Champion Rehabilitation and Nursing Center in Massachusetts seeks a registered nurse to lead and support MDS assessments and care planning in a busy long-term care setting. Daily Pay option available!!

You will collaborate with a clinical team to ensure accurate documentation, timely care plan updates, and resident-focused care while maintaining confidentiality and strong teamwork in a friendly, family-like environment.

Qualifications

• Valid RN license in Massachusetts.
• MDS experience required.
• Long-term care experience required.
• Highly organized, professional and motivated.
• Solid computer skills.
• Excellent communication and teamwork.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.

Requirements:

• Valid RN License
• MUST HAVE PRIOR MDS EXPERIENCE
• Long Term Care Experience Required

• Should be friendly and a team worker
