Job summary

Champion Rehabilitation and Nursing Center in Brockton, MA is seeking a qualified RN with MDS experience to join our team. You will complete assessments, care plans, and monthly calendars, update records, and maintain documentation to ensure quality resident care.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting confidentiality of resident and facility information at all times

Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned

Requirements:

• Valid RN License


• MUST HAVE PRIOR MDS EXPERIENCE
